The Ministry of the People, Social Development and Family Services says it is taking a structured approach to tackling social displacement through its Continuum of Care framework.
Parliamentary Secretary Natalie Chaitan-Maharaj said it is designed to support vulnerable individuals from initial engagement through to rehabilitation.
“The continuum of care framework provides a structured needs-based pathway from initial engagement through to rehabilitation and, where feasible, reintegration into independent living. Its key elements are outreach and voluntary engagements, assessment and temporary care, including emergency accommodation, stabilisation, counselling and development of an individual care plan, primary care, including specialised healthcare and mental health services, substance use treatment, rehabilitation and life skills support, advanced care through transitional housing, supported long-term care or independent living arrangements, and monitoring and follow-up.”
She said the Ministry is also reviewing its existing legislative and policy framework on social displacement.
“The Ministry has established an internal technical committee to review the existing legislative policy and institutional framework governing the national response to social displacement and to identify areas requiring strengthening or modernisation. The review includes the adequacy and operational status of current legislative provisions, definitions, institutional responsibilities, service delivery arrangements and referral mechanisms. The committee is also advancing work towards an integrated national framework for social displacement covering the full continuum of care from prevention, identification and outreach through assessment, emergency accommodation, treatment and rehabilitation, transitional support, reintegration, monitoring and follow-up..”
